Elective Hematology/Oncology/BMT Rotations

Non-Standard/Outside Fellow Process

Rotation Restrictions

  1. Elective rotations will not be allowed during the months of June through August
  2. Rotations will only be granted for inpatient services (Hematology Consult Service, Oncology Consult Service, Bone Marrow Transplant/Leukemia)

 

Eligibility Requirements

You must meet all of the following requirements before submitting your application:

  1. Requests must be submitted a minimum of 90 days before requested rotation date
  2. Applicant must be PGY-2 or higher
  3. Background check and drug screen within 6 months prior to requested rotation
  4. Rotations cannot exceed 90 days
  5. CV and Photo
  6. Copies of current immunization records including flu vaccination
  7. Proof of Malpractice Insurance
  8. BNDD registration to reflect rotation
  9. Accepted applicant will be required to attend didactics/lectures and selected tumor board meetings
  10. Letter of reference from fellowship program director
    • (a) Recommend fellow for rotation
    • (b) Fellow meets ACGME milestone requirements
    • (c) Confirm excellence in professionalism and absence of serious conduct concerns

 

Approved Rotation Request

  1. You will receive notification of approval within 10 business days of request (Hematology/Oncology Fellowship Program reserves the right to deny rotations)
  2. You will be required to provide the following requested documents within 5 business days; you will not be reminded. If the documents are not received within 5 business days, your application for rotation will be withdrawn.
    • (a) Signed Affiliation Agreement (required to be signed and returned 60 days prior to rotation request)
    • (b) Washington University Non-Appointee Access Statement
    • (c) Washington University and Barnes Jewish/Christian Healthcare Confidentiality Statement
    • (d) Washington University Non-Employee Personal Information Form
